FOR FRESHMEN
» Form 138 or Fourth Year High School Card
» Principal's Certificate of Good Moral Character
» Principal's Certification of Academic Ranking for Valedictorians and Salutatorians.
» Original and Xeroxed copies of Birth and Baptismal Certificates
» Six (6) copies of 1x1 ID pictures
» Medical Certificate
» Police/NBI Clearance for those coming from outside the province.


FOR OLD STUDENTS
» Class Cards of the previous term. If the class cards have been misplaced or lost, certification from teachers concerned must be secured.


FOR TRANSFEREES
» Transfer Credential (Honorable Dismissal)
» Xerox copy of scholastic records
» Satisfactory entrance examination result
» Letter of Recommendation from the Dean/Guidance Counselor
» Birth and Baptismal Certificates
» Medical Certificate
» Six (6) copies of latest pictures (1 1/2 X 1 1/2)
» Police/NBI Clearance for those coming from outside the province


FOR CROSS ENROLLEES
» Cross registration permit from the Dean/Registrar of the student applicant's home school
» Two (2) copies of 1 1/2 x 1 1/2 pictures

FOR ALIEN STUDENTS
Official transcript duly authenticated by the Ministry of Educationof the student applicant's country
» Personal history statement (forms are supplied by the College)
» A notarized affidavit of support
» Study permit and Alien Certificate of Registration (CAR)
» Certificate of English Proficiency for applicants from non-English speaking countries
» Satisfactory entrance examination result
» Six(6) copies of latest picture (1 1/2 x 1 1/2)

FOR MILITARY (Veterans Beneficiaries) PERSONNEL DEPENDENTS
» Pursuant to the Presidential Decree No. 577, dependent of Philippine Military Personnel who died or have become incapacitated in the line of duty are granted complete exemption from tuition and matriculation fees
» True copy of Entitlement to Veterans' Benefits


PROCEDURE FOR FRESHMEN ENROLMENT
» Get your credentials from the Human Development Center

» Get the enrollment form from the College Chairperson or College Chairperson’s Secretary at the Chairperson’s Office.

» Properly accomplish the enrollment form and have it signed by the College Chairperson. (Note: Copy only the subject codes prepared by the Registrar’s Office (not the course description). You may ask the help of the Chairperson or Chairperson’s Secretary regarding this)

» Present the enrollment form together with the credentials to the School Registrar for verification at the Registrar’s Office

» Have the enrollment form signed by the School Registrar

» Have the assessment of fees at the Finance Office

» Pay the required fees for enrollment at the Finance Office

» Proceed to the Studio Room near the Printing Office (Basement Rooms) for your ID picture taking
