  • Photocopy of the following
    :: birth certificate (clear copy)
    :: baptismal certificate (clear copy)
    :: marriage certificate

  • Dental clearance
  • Picture (2 x 2) 4 copies
  • Entrance Test and Interview
    • * Please bring pencil


FOR TRANSFEREES ( GRADE 1 TO 10)
  • Report Card ( Form 138)
  • Photocopy of the following
    :: birth certificate (clear copy)
    :: baptismal certificate (clear copy)
    :: marriage certificate

  • Letter of Recommendation from:
    • :: Class Adviser
    :: Principal/Guidance Counselor
    :: Parish Priest

  • Dental clearance
  • Picture (2 x 2) 4 copies
  • Entrance Test and Interview
    • * Please bring pencil


PROCEDURE
  • Complete the required admission requirements.

  • Get an application form from the BED Office

  • Accomplish the application form then file it at the BED Office

  • Upon approval of the application, pay the testing Fee of P100.00 at the Finance office

  • Take the entrance Test on the scheduled date at the Guidance Office

  • Report for interview as scheduled

  • Get the Test and Interview results at the BED Office as scheduled

    • *NOTE: if the applicant is admitted, he/she is given the reservation slip for payment at the Finance Office, P500.00
